What Outreach Offers

The Outreach service is designed to offer flexible support on a weekly basis from 2 hours a week to 12+ hours a day - sleep over or waking night shifts can also be provided if necessary. Outreach can be delivered within your own or your family home or out in the community. Days, times and the frequency of visits are entirely flexible to suit your lifestyle and needs. 


Outreach offers an alternative to supported living services or a Personal Assistant (P.A.)


The Outreach service offers a regular team of well trained staff who can provide you with a number of day to day tasks which you may need help with (as agreed in your Support Plan).


Including -

Support in the home, with things such as 


  • personal care
  • administering medication
  • home making skills - cleaning, laundry, meal preparation
  • weekly budgeting
  • easing isolation


Support outside of the home, with things such as


  • going shopping
  • going to the bank or post office
  • attending appointments
  • visiting family and friends
  • using local amenities 

Support to access activities, work and

educational pursuits, including -


  • help to develop ideas and plan your timetable
  • help to budget for activities or apply for funding/free placements 
  • help with transport
  • support to attend activities and to encourage participation

Support with other things, such as –

  • monitoring your health and wellbeing – and reporting concerns to the relevant people
  • encoraging healthy eating and supporting you to excercise (where necessary)
  • planning necessary appointments
  • managing your finances – making sure you are getting the correct benefits, paying your bills, banking
  • applying for benefits or benefit reviews
  • helping you to find appropriate accommodation and supporting you to move
  • maintaining and encouraging family and social connections
