If you would like to know more about commissioning our Outreach service, the first step is to contact us. You can either do this directly or via your Social Worker.
You can contact us by phone, letter, email or by the direct contact form in the ‘Contact Us’ section.
We can discuss your basic needs over the phone or by email or letter. We can then organise a meeting, either at our offices or at your home – whichever is more convenient.

After the details have been agreed, we can introduce you to your care team. You may want us to organise a number of short visits before your support package is agreed fully – to make sure your new staff know you well and you like the support you are getting.
When you are happy with everything, a start date can be agreed.
