As an intergral part of the consultation process, we sent out an easy read questionnaire to all of our service users. Each question was designed with careful consideration to the five key areas -
To gather feedback from families, we held a consultation meeting on the 1st July.
The meeting was attended by the management team, team leaders and a number of family members (everyone had been invited but some could not attend).
Overall, the event was very positive with good constructive feedback which we will be including in our self assessment and using to compile our final action plan.
For those parents/families unable to make it, we sent out service questionnaires to ensure they could still provide us with feedback.

To ensure staff could also contribute to the consultation, Team Leaders were invited to attend the consultation so they could cascade information both ways. Manager's also asked a number of generic questions to during monthly meetings to gauge feedback from members of each Care Team -
